How to Apply

IDVR’s preferred method for accepting applications is through a series of steps: 

1. The individual has completed the Request for VR Services form below;
2. Complete a virtual intake appointment with an IDVR staff member;
3.  Sufficient information has been obtained for IDVR to initiate an assessment for eligibility and priority for services;
4. The individual is available to participate in the eligibility determination process; and
5. A signed IDVR Application has been received. 

An individual may skip these steps prior to applying for VR services and provide IDVR with a completed and signed  IDVR Application. However, delays in providing information needed by IDVR may impact the agency’s ability to determine eligibility and priority for services within federally required timeframes.
